3D Lifestyle Membership Terms and conditions apply to all users who subscribe to the unique 3D Lifestyle membership plans as posted on our website, web application and sold through 3D Lifestyle Medi-Spa locations.
3D Lifestyle offers several membership plans and clients can select the membership plan that fits their needs.

- Exclusive and preferred pricings on all treatments
- Preferred scheduling and availability of time slots to members
- Exclusive availability of treatments, specials and offers to only 3D lifestyle members
- No booking deposits and No-show fees for 3D lifestyle Members
3D Lifestyle Membership - Exclusive Membership to Aesthetics and Wellness Luxury
Applicants for membership must be at least 18 years of age. Anyone under the age of 18 must have parental consent.
Upon payment of the 3D Lifestyle membership plan fees, benefits become accessible to over 150+ Medical Aesthetics and Wellness Treatments and Experiences. 3D Lifestyle’s access to discounts, promotions, and special services or treatments are included with membership. All membership fees are paid in advance prior to the start of the period of membership.
3D Lifestyle Membership offers a unique model where monthly Membership fee is converted into 3D Lifestyle Electronic Wallet Money and added to Member’s Wallet on the member’s 3D Lifestyle online platform. Members can use the electronic Wallet points to pay for various treatments in Full or in part. Membership fees can be accumulated and used at once to pay for any of the treatments available in a 3D Lifestyle Spa. Unless otherwise specified by 3D Lifestyle upon registration, the Membership Fee covers one month at a time and is renewed every month on the same day as the Membership start date. Each Member explicitly consents to automatic renewal of Membership and associated costs; however, a Member may opt-out of automatic renewal ten (10) days prior to the renewal date by informing 3D lifestyle and confirming the cancellation of renewal.

Restrictions may apply between various membership plans.
Minimum Term of 3D Lifestyle Membership
A minimum of 12 months commitment is required for any membership plan.Right to update and make changes
3D Lifestyle holds the right to update or change the price and features of its Memberships at its sole discretion. 3D Lifestyle has the right to periodically examine subscriptions. Members shall be notified in advance of any changes, including any increase in membership fees and any change in the date of automatic membership renewal and membership dues withdrawal.Product Brands
The product brands used during treatment sessions vary based on available stock and market availability. Even if previously advertised, we cannot guarantee the availability of any product during appointment scheduling and treatment.
All products are Health Canada-approved, and the patient has the option to decline treatment with a particular brand or kind of product. If the client declines treatment prior to the commencement of the procedure, the appointment is canceled and the payment is refunded in full.
The 3D Lifestyle clinics are not responsible for any losses or damages that may have happened as a result of a client canceling an appointment. After the procedure has begun, treatment payments are neither refundable in full nor in part.

Termination Or Suspension Of Membership
3D Lifestyle retains the right to revoke or suspend a member’s membership at any time for the following reasons:
- The member violates and/or abuses 3D Lifestyle Membership Terms.
- If membership dues are overdue by thirty days.
- If a membership credit card on file has been declining for over 3 months, one’s membership will be canceled without notice and the e-wallet forfeited.
- If a member cancels their membership prior to 12 months commitment, 3D Lifestyle will void all electronic Wallet points without notice.
Canceling Membership
- A member can cancel their membership anytime after 12 months.
- Members have the right to cancel or suspend their Membership 10 days prior to their renewal date. Members can request their default clinic in writing to suspend or cancel their membership. Alternatively, members may also email care@3dlifestyle.ca and include their full name, email and phone number along with the request to cancel or suspend their membership. 3D Lifestyle team will provide a written notice of confirmation that their membership has been canceled/ suspended.
- 3D Lifestyle has the right to modify, add, or remove certain services and facilities without prior notice, as well as the right to change facility hours.
- A client with a canceled membership shall have six months from the date of the membership cancellation, to use all the wallet points in their eWallet against treatments and services. After the 6 months period, the eWallet shall be forfeited.
